MyMeldRx

It serves as the personal health data repository that you completely control. You decide what health data sources to connect and who can view or download that data.  

 
 

1. Connect and Download Data

MyMeldRx™ app enables you to connect with any FHIR® enabled source that houses your healthcare data and store it in a centralized repository that you control. Download data from your Medicare account, other health plans, EHRs, patient portals, labs, pharmacies, and fitness trackers.

 

2. View and Manage Data

View your health information in an easy-to-read format and keep track of your medications, conditions, doctor visits, procedures performed and other fitness parameters. 

Refresh the data regularly to keep the information up-to-date.

 

3. Share the Health Data

Share your health data easily and securely with family members, caregivers, providers, and research organizations, and contribute to population health efforts.

You control what data you share and who you share it with.

To share the data:

  • Select the data you wish to share.

  • Send a secure Download Invite to the entity you wish to share it with. 

  • The selected data will be stored securely in the cloud as a FHIR Bundle till it is retrieved by the intended receiver.

 

4. Connect to Other Apps

MyMeldRx lets you connect with any app whether it is SMART® enabled or not.

Connect your health data with apps of your choice to track and manage chronic conditions, boost fitness and improve your health.
